West Hartford-Bloomfield Health District to hold two free H1N1 vaccinations in February

WHBHD H1N1 Vaccination for ages 18 years and older, February 4 at 10 A.M. to 12 noon
This clinic is intended for residents of West Hartford and Bloomfield or persons working in West Hartford or Bloomfield. Please complete the required pre-registration information for the WHBHD sponsored Influenza A (H1N1) clinic for persons age 18 years and older on Thursday, February 4, 2010 from 10 A.M. to 12 noon at Saint Mark's Parish Center, 467 South Quaker Lane, West Hartford, CT. Vaccine will be injectable containing thimerosal. The vaccination is FREE. The vaccine supply is limited. Pre-registration is required. If you need assistance completing the on-line pre-registration please call (860)561-7900 and choose item 4 on the automated voice menu. At the clinic you will be required to complete and sign a consent form.

Vaccine will be injectable containing thimerosal. The vaccination is FREE. Vaccine supply is limited. Pre-registration is required. Please complete the following pre-registration form. If you need assistance completing the on-line registration you may call (860)561-7900 and select item 4 for the automated voice menu. At the clinic you will be required to complete and sign a consent form. A parent or guardian must accompany persons under 18 years old.
